Chronic Disease Management via Telehealth in Missouri

Living with a long-term condition means regular check-ins, medication reviews and lab results that need attention. These visits give you set time to review how things are going, adjust your plan with Dr. Seavey and ask questions, without the drive to an office.

Questions

Common questions

Have a question that is not answered here? Call 573-217-6804 or email [email protected].

How often will I have visits?

That depends on your condition and your plan. Dr. Seavey will recommend a follow-up schedule with you.

Should I track readings at home?

If you have a home blood pressure cuff or glucose meter, bringing recent readings to your visit helps you and Dr. Seavey review your plan together.
